Here’s a simple list of information you may need to file your claim:
- VA Form 21-526EZ (Application for Disability Compensation and Related Compensation Benefits);
- Medical Records from your doctor(s) about the injury or condition;
- Medical and Personnel Records from your military service, including DD Form 214 (Certificate of Release or Discharge from Active Duty) and DA Form 3349 (Physical Profile), which you can request online from the National Archives;
- Letters from Servicemembers you served with who may have seen you receive the injury or known of the injury or condition during your time in active duty; and
- Supporting letters from family, friends, or other people with knowledge of your injury or condition.
All claims are unique and may require more than the documents in the list above.
